59M with lymphoma, day 10 post-chemotherapy, admitted this evening with febrile neutropenia (side room 3), end of your evening shift.

• Febrile neutropenia: T 38.4 at 21:00, neutrophils 0.3; first dose of piperacillin-tazobactam given at 21:50.
• Next antibiotic dose at 05:50; it must not be missed or delayed.
• If he develops hypotension, rigors, or a temperature above 39: escalate immediately, repeat cultures, and call the hematology registrar on call.
• Blood cultures drawn from the port and peripherally before antibiotics; results will flag to the ward.
• He is on protective isolation; strict hand hygiene, no fresh flowers in the room.
• His lymphoma is stage III, diagnosed four months ago.
• He prefers to be woken for observations rather than have them skipped.
• Day 10 is his expected count nadir; counts will be repeated on the morning bloods.
