• New AF today at rates up to 142; rate-controlled on oral beta-blockade since noon, last HR 88.
• If HR sustained above 120 or any chest pain, hypotension, or breathlessness: ECG, and call me or the on-call medical registrar.
• Potassium and magnesium were replaced today; repeat potassium due at 22:00, keep above 4.0.
• Anticoagulation is deliberately NOT started: decision pending with the surgical team tomorrow because of bleeding risk. Do not start heparin tonight.
• She had bowel surgery on Monday with Professor Okafor.
• Her AF was probably driven by hypovolemia and low potassium.
• ECG from 01:30 and from noon are both in the chart for comparison.
• Her daughter phoned twice today asking for updates.
• She is eating and drinking normally again as of this evening.
