83F on warfarin who hit her head in a witnessed fall at 17:30 (bed 26), CT normal, handing over at 20:00.

• Witnessed fall 17:30 with head strike; CT head at 18:40 normal; INR 2.7; warfarin for AF, tonight's dose HELD pending the day team's review.
• Neuro observations 2-hourly until 06:00; any new headache, vomiting, confusion, or GCS drop: repeat CT immediately, do not wait for morning.
• She is on the falls pathway: bed low, call bell in reach, supervised walking only tonight.
• Fall looked mechanical (slipped reaching for her stick) but the morning needs the full falls workup: lying-standing pressures, ECG review, medication review.
• If she needs analgesia, paracetamol only: no NSAIDs on warfarin.
• Her INR target is 2 to 3 for the AF and it has been in range for months.
• Daughter has been phoned and knows about the fall.
• She normally walks with a stick and lives in sheltered housing.
• She dislikes the hospital socks and has her own slippers.
• Physiotherapy referral has been sent for the morning.
